東部某區域醫院停經後婦女身體質量指數、主觀骨骼肌肉症狀與骨質密度之探討

目的:許多研究已證實骨質疏鬆對於停經後婦女,常常造成健康上的威脅。因此,本研究的目的主要為運用回溯性研究法,調查台灣東部某區域教學醫院停經後健康婦女,主觀的肌肉骨骼症狀與骨質密度程度之相關性為何,藉以了解特定族群在此健康問題之分佈情形。
方法:本研究採用回溯研究法,資料來源為2011年1月至12月底,就診於東部某區域教學醫院家醫科門診,進行身體健康檢查的停經後婦女為收案個案,人數有215人,除了基本資料之外,骨質密度預計以雙能量X光吸收儀(Dual energy X-ray absorptiometry),檢測腰椎第1至4節前、後徑及左髖部數值,而骨質疏鬆症定義依據世界衛生組織的診斷標準,再以登錄表記錄個案的身體質量指數(Body Mass Index, BMI),骨質密度(Bone mineral density,BMD)與主觀骨骼肌肉症狀等數據。所有的數值採SPSS 17.0進行統計分析,以描述性統計、卡方檢定、Pearson’s相關、ANOVA進行分析與處理。
結果:本研究結果發現年齡層愈高,則與T- score(F=14.85, p <.05)、spine-BMD(F=14.82, p <.05)與hip-BMD (F=20.74, p <.05)等呈反比。身體質量指數與脊柱骨質密度呈現相關性(p<.05);臨床肌肉骨骼症狀與年齡、T-score、Z-score、spine-BMD和hip-BMD有相關性,但與BMI、身高、體重無相關;T-score、BMD會隨著年齡、身高、體重不同而有差異性。
結論:因此,本研究證實年齡與身體質量質數的確會影響個案的骨質密度,甚至造成停經後婦女的肌肉骨骼症狀,因此,研究結果期望提早給予此特定族群適合的護理衛教及處置,減緩停經婦女的骨質疏鬆與骨骼肌肉症狀等情形,藉以提升生活品質。

Objectives
Many studies have confirmed osteoporosis for postmenopausal women often causing a health threat Therefore the purpose of this study is mainly the use of retrospective research method to investigate the eastern part of Taiwan in an area Hospital of postmenopausal healthy women subjective musculoskeletal symptoms and bone density and extent of the related nature why in order to understand the distribution of a particular ethnic group in this health problem of the situation
Methods
Data was collected from January 2011 to December in Family medicine clinic of a teaching hospital located in eastern Taiwan Preliminary estimate of about number of 215 health menopausal women were enrolled in this study Bone mineral density (BMD) of L1- L4 lumbar spine and left hip was measured by dual energy X-ray absorptiometry Data of Bone mineral density(BMD) body mass index (BMI) and musculoskeletal symptoms were collected from medical records Osteoporosis was diagnosed in accordance with the criteria of World Health Organization All the values was adopted SPSS 17.0 statistical analysis descriptive statistics chi-square test Pearson s correlation ANOVA analysis and processing
Result
The results of this study showed the age was inversely proportional to the T score(F=14.85, p <.05) spine-BMD(F=14.82, p <.05) and hip-BMD(F=20.74, p <.05) There was significantly difference in BMI and spine-bone mineral density in social demography; And bone density was positively correlated with age, clinical symptoms of skeletal muscle, T-score, Z-score, spine-BMD and hip-BMD There was not significantly difference in age and weight
Conclusions
In this study we found that the age can effect the bone density and muscle skeletal symptoms in postmenopausal women Therefore we hope to provide the optimal education and nursing care about the prevention osteoporosis and decrease the symptoms of skeletal muscle for improve the quality of life
